Easy To Use |
It's very simple to use. The member can use our Website to locate a provider nearest to them. They may call the 800 number on the back of their membership card or call ahead to the provider to verify the provider is definitely in our Network. The member simply presents their membership card to the provider each time they go in for their visit or with each prescription to be purchased. The provider calculates the discount and the member pays the discounted price. No forms required … It’s that easy. |

Savings may vary by provider and geographical location and are subject to change. Members are obligated to pay for all health care services and will receive a discount from those health care providers who are contracted.
This Discount Card carries a 30 day cancellation period.
